WORKING WITH OUT LOUD
UNDER A NDIS PLAN

Out Loud enjoys working with families accessing support through their child's NDIS plan.  hwen working with families under a plan, we required some basic details to ensure we meet NDIS requirements as well as assisting families in recognising key timeframes and budget management.  Families are encouraged to provide all details of their NDIS plan & goals to help us with therapy planning and provision, however as a minimum we require:

  • NDIS number

  • Plan dates

  • Management Plan style (Self, Plan, Agency)

 

When commencing services with Out Loud, we will draw up a Service Agreement for the duration of your plan.  Service Agreements can be updated as needed to adapt to the therapy needs of your child, but allows us to document our commitment to each other. The Service Agreement can be used to update your contact details, provide consent for Out Loud to communicate with third parties on your behalf, and outlines our cancellations and non-attendance  policies. The final page of the Service Agreement is the Schedule of Supports, which will list each service and its cost.
